Kakau Inoa Registry

The 2010 Census indicates that more than a half a million individuals living in the United States identified themselves as Native Hawaiian. It also indicated that 50% of that population lives outside of the State of Hawai’i. KAKAU INOA – Sign Your Name – is an attempt to identify and locate the largest part of that 50% believed to be living on the continental U.S. and Alaska. The data collected will be used to ascertain needs and best practices for improving relevant statistics that will enable Native Hawaiians to benefit from existing programs specifically designed “for the betterment of Native Hawaiians” no matter where they live. Instructions for Registration Form
Please read these Instructions completely. They will answer many of your questions.

Privacy Policy: Your personally identifiable information will not be disclosed or used for purposes other than those specified by your consent. Your information will be kept in a secure database not accessible remotely or from the internet. This form is for indigenous people of Hawaiian ancestry only. There is no age requirement. The blood quantum can be any amount but must be verified and traceable to their Hawaiian ancestry. All registration forms must be accompanied by copies of verification of Hawaiian ancestry such as birth, death or marriage certificate(s). Use one registration form per person. Fill out the form completely. If you are filling out the form for yourself, fill in section “F.” If you are filling out the form on behalf of any other person, skip section “F” and fill in section “G.” You must have the registrant’s consent to fill this form out for them. Do not fill in any of the items in the block at the bottom. This is for official use only and is used to track the completeness of your application. Incomplete forms will be tracked to verify missing data. If the information on the form is incorrect or outdated and we cannot contact the person filling out the form to get the missing information the registrant’s name may be removed from the registry.

After filling out the form completely, print it out and sign it. Don’t forget to save it and make a copy for yourself. If you fill out forms for other family members use the same redundant data from the previous form and “save as” the other person. In this way, you can use the same information where it is appropriate and change where you need to and still have copies of each. Don’t forget to sign in section ”G” when you fill this out for someone else.
