The 2010 Census indicates that more than a half a million individuals living in the United States identified themselves as Native Hawaiian. It also indicated that 50% of that population lives outside of the State of Hawai’i. KAKAU INOA – Sign Your Name – is an attempt to identify and locate the largest part of that 50% believed to be living on the continental U.S. and Alaska. The data collected will be used to ascertain needs and best practices for improving relevant statistics that will enable Native Hawaiians to benefit from existing programs specifically designed “for the betterment of Native Hawaiians” no matter where they live. Please read these Instructions completely. They will answer many of your questions.
After filling out the form completely, print it out and sign it. Don’t forget to save it and make a copy for yourself. If you fill out forms for other family members use the same redundant data from the previous form and “save as” the other person. In this way, you can use the same information where it is appropriate and change where you need to and still have copies of each. Don’t forget to sign in section ”G” when you fill this out for someone else.
